SVMS Athletic Sport Form
All SVMS student athletes must complete the Athletic Sports Form prior to tryouts. You will not be allowed to tryout for any sport until the form has been completed. If you are planning on trying out for more than one sport this year please indicate which sports you are interested in on the form. With this new system you will only have to fill out one form annually.

EXTRA CURRICULAR CODE OF CONDUCT
In order to participate in extracurricular activities, including but not limited to, sports, school-sponsored clubs, and Student Government, a student and their parents/guardians must understand and agree to follow the rules below. Any student/parent/guardian who does not follow these rules is subject to suspension or removal from the team or organization.
Students must abide by the district policies related to discipline.
Any student suspended from school will be removed from any meetings, practices, or sporting events for the suspension period. The use of alcohol or illegal drugs on or off campus: Any student caught using these substances will be immediately removed from the team for the remainder of the season.
Students must have regular and prompt attendance at all practices and games, except in the case of illness or when the coach excuses the student.
Students/parents/guardians must follow the rules established by the coach, teacher, or director of the program.
Students/parents/guardians must represent the school and community through good citizenship and conduct, including but not limited to:
Displaying cordial conduct to visiting team members
Displaying good sportsmanship
Respecting the integrity and judgment of athletic officials, coaches, and athletic directors and avoiding divisive action or words, towards them.
Not approaching coaches/officials/directors during practice or games.
Avoiding profanity, vulgarity, racist comments and rowdiness, and obscene actions of any nature.

Attendance:
Anyone absent from school on game/activity/event day is not eligible to compete/participate. They must be in attendance for at least 4 periods of the day to be eligible for competition.
A student who is absent from school will not participate in practice on that day.
A student is expected to attend every practice and get permission from their coach/teacher if they cannot make practice.
Students must be able to participate in PE class to be eligible for after school athletics. Any student who is excused from PE by a parent or a doctor’s note, will be unable to participate in athletics for the same duration.
The google form will be provide to the parents email once the student has made the team.

Dear SVMS Athletic Parents,
After-school sports are essential to the growth and development of many students. Lessons learned from participating in school athletics are priceless—these include learning new skills, discipline, camaraderie, school spirit, and a sense of belonging.
Scotts Valley Middle School’s after-school sports program exists because of parent support; we receive no district funding for athletics. It costs approximately $125 per athlete, per sport to run each team, covering essential expenses like referees, tournaments, coaches’ stipends, equipment, and uniforms. This amount, which had not increased in over two decades, has been adjusted this year to keep pace with rising costs and ensure we can continue offering a high-quality athletic experience for all students. Your contribution directly impacts your child’s opportunity to compete, grow, and thrive as part of a team.
We value our students and their activities, and we know your time is valuable. Instead of multiple fundraising efforts, we ask that you donate your tax-deductible contribution to help support your child in our award-winning athletic program. If paying by check, please make it out to SVMS Athletics and indicate your child’s sport in the memo section.

To ensure a successful season for all, we will be hosting mandatory parent meetings before each season for parents and students of all teams. At these meetings, team expectations will be clearly defined.
