  • All Symptoms

  • Date of onset of illness

Student must have a negative PCR test before returning to campus (see Test Off-Campus for locations)

DID YOU GET AN EXPOSURE NOTICE?

SIf you student is symptomatic... keep them home, notify school, and schedule a test. (see Test Off-Campus for locations)

If your student is asymptomatic and:

  • Unvaccinated.. they can stay in school if they get tested at the next on-campus PCR testing date. (see Test On-Campus)

  • Vaccinated... they can come to class without testing.

QUARANTINE STUDY

Available to students under a 10-day Home Quarantine or for students who are symptomatic and isolating at home for more than 5 days.

TESTING PERMISSION

AFTER you have signed up with Inspire Diagnostics, if you ONLY want SVUSD to have permission to utilize the PCR testing for EXPOSURE / EVENT TESTING ONLY, please complete this survey.

If you want your student to be included in BOTH surveillance and exposure/event testing using PCR testing, please register with Inspire Diagnostics and DO NOT COMPLETE THIS SURVEY.
