jperry@scottsvalleyusd.org  to begin a new student registration.

To begin registration process for a NEW STUDENT (or returning to our district after attending another school) we will need:

1.  Age Verification – the following documents will be accepted:
   • Certified copy of birth record
   • Statement by the local registrar or county recorder certifying the date of birth
   • Baptism certificate
   • Passport
   • When none of the foregoing is obtainable, an affidavit of the parent, guardian, or
   custodian, or any other appropriate means of proving the child’s age.

2. Current Immunization Record

3. Address Verification – two of the following documents will be accepted:
   • Property tax payment receipts
   • Rental property contract, lease, or payment receipts
   • Utility service contract, statements, or payment receipts
   • Pay stubs
   • Voter registration
   • Correspondence from a government agency
   • Declaration of residency (English),(Spanish) executed by the parent or legal guardian of the student.

4. Caregiver's Authorization Affidavit - required only if you are not the student's legal guardian and the student is living in your home.

PLEASE NOTE:

5th grade students coming from Vine Hill and Brook Knoll:

Your child is considered a "Returning Student", a "roll-up" within the SVUSD district. They have a 6th grade enrollment at Scotts Valley Middle School. You will be asked to complete a "Back to School" update in your SYNERGY parent portal in August.

Families who have never had a student attend a Scotts Valley Unified School District school are required to create a ParentVue account to register students online. You will need a valid email address in order to create a ParentVue account.

Once you are able to login into your ParentVue account, click "Online Registration" in the upper right corner and choose "2025-26 New Student Pre-Registration" from the drop down menu.

• Information regarding REG and WEB days will be posted on our home page summer of 2025.

• You will be asked to log into your SYNERGY parent portal and complete the annual SVUSD "Back to School"  forms online during the summer of 2025.
