If you leave a voicemail or email be sure to include:

  • Students full name

  • All Symptoms

  • Date of onset of illness

  • Call back number

Student must have a negative PCR test before returning to campus (see Test Off-Campus for locations)

Did you get an exposure notice?

SIf you student is symptomatic... keep them home, notify school, and schedule a test. (see Test Off-Campus for locations)

If your student is asymptomatic and:

  • Unvaccinated.. they can stay in school if they get tested at the next on-campus PCR testing date. (see Test On-Campus)

  • Vaccinated... they can come to class without testing.

Quarantine Study

Available to students under a 10-day Home Quarantine or for students who are symptomatic and isolating at home for more than 5 days.

SVMS Quarantine Study Form: Provide the school a signed copy of the first page.
